1.  Completed Georgia State Merit Application
2.  Valid Georgia Driver's License
3.  Social Security Card
4.  Certified Birth Certificate
5.  Proof of High School Diploma or GED
6.  Test Scores (ACT, SAT, Compass or Accuplacer) or proof of 4-year degree from an accredited college or university
7.  DD214 long form with indicated discharge (Military Personnel Only)
8.  POST Certification (if applicable)
9.  Naturalization forms if Non-US citizen
